Bilingual Community Health Worker – Primary Care Outreach to Seniors
Regular full-time position

Under the general direction of the Primary Health Care Program Director, and as a member of the Primary Health Care Team, the Community Health Worker (Primary Care Seniors Outreach) is responsible for working within the frameworks of community development and health promotion, and using the health links approach, to provide community health services to clients registered with the Primary Care Outreach for Seniors program.  The incumbent will provide health education, advocacy, practical assistance and support, access to other programs and services, and assist clients and their caregivers to build support networks by working closely with a variety of community and internal partners, especially the Primary Health Care, Health Promotion, and Counselling Teams.

  • Undergraduate degree in a relevant discipline from a recognized university or combination of some post-secondary education and significant experience in health promotion and/or community development.
  • Three to five years full-time relevant experience in a community setting.
  • Minimum two years full-time experience working with seniors.
  • Experience in providing community-based services including crisis intervention, group work, referral and advocacy.
  • Thorough knowledge of and proficiency in current assessment and solution-focused intervention techniques.
  • Thorough knowledge and understanding of relevant community resources.
  • Ability to assess situations and to respond appropriately.
  • Interest in a variety of health issues and willingness to learn and teach others about them.
  • Experience in health promotion including community development.
  • Excellent communication and interpersonal skills.
  • Experience in program development, implementation, monitoring and evaluation.
  • Ability to work collaboratively in a multi-disciplinary setting, with clients and other agencies who are involved with the care of the clients.
  • Demonstrated organizational skills.
  • Ability to work flexible hours based on client need.
  • Sound judgement and strong problem-solving skills.
  • Demonstrated proficiency with computer systems, including word processing, spreadsheets and email software.
  • Demonstrated experience with data management systems in a health setting
  • Valid driver’s license, insurance and regular access to a vehicle for client transportation
  • Under Carlington’s designation to provide French Language Services, this position requires the following proficiency in the French language:
      • French Oral Expression: Advanced
      • French Oral Comprehension: Advanced +
      • French Reading: Advanced

Salary Range: $25.43 – $29.91 per hour + benefits
